This document outlines the Terms and Conditions for all scholarships, bursaries, and awards administered by the BLF Foundation Scholarship & Mentorship Program. All scholarships, bursaries and awards will be referred to as “scholarships” herein. The BLF Foundation Mentorship Program will be referred to
as “mentorship” herein. You, the student applying for the Scholarship and Mentorship Program, will be referred to as “applicant” herein. By accepting the BLF Foundation Scholarship and/or Mentorship Program, you are agreeing to these terms and conditions.
You must read these Terms and Conditions in its entirety before you accept your scholarship and/or mentorship offer. When you accept your scholarship and/or mentorship, you are agreeing to and accepting these Terms and Conditions and agree to
the requirements and conditions of your scholarship and/or mentorship.
Applicants in receipt of a scholarship will be required to sign the Scholarship & Mentorship Individual Agreement, herein referred to as the “Agreement”, form at the end of the Terms and Conditions. Applicants must meet all conditions as outlined in the agreement. Applicants in receipt of a scholarship will also abide by the Mentorship Program Terms and Conditions.
Applicants in acceptance of mentorship will be required to sign the Mentorship Individual Agreement form at the end of the Terms and Conditions. Applicants must meet all conditions outlined in this agreement.
All correspondences between the applicant and the BLF Foundation will be operated primarily via email, with the option of telephone communication or communication based social media applications, based on the BLF Foundation decision.
Any successful applicant may be required to show proof that one is enrolled in, or have been accepted in accredited university course, commencing studies for the spring semester 2017 and will need to show studies for the fall semester 2017 upon summer 2017 registration. For a graduating senior in spring 2017, only spring semester 2017 registration will be needed.
All applicants offered scholarships will receive an official ‘offer’ email offering the BLF Foundation Scholarship Award and Mentorship Program, and must reply either accepting or rejecting this offer within 7 days of receipt.
All applicants offered mentorship will receive an official ‘offer’ email offering participation in the program, and must reply either accepting or rejecting this offer within 7 days of receipt.
Failure to respond within this 7-day period may result in the scholarship or mentorship offer being withdrawn and reassigned. You must accept your scholarship or mentorship by the due date, as instructed in your offer email, to be eligible to receive payment and/or be active in the program.
The BLF Foundation will make payment of the funds for the scholarship in one single installment, though we reserve the right to make payment in multiple installments, as we see fit. All technology awards will be given at either the first or second mentor session, dependent on shipping timeline provided by our technology vendor.
The total value of the scholarship offered will be outlined in the official offer email. We reserve the right to offer any value of funds, up to the value of $1000.
The scholarship funds will be sent directly to your educational institution of higher learning. A check will be sent including your school ID to ensure proper financial payment application. The BLF Foundation reserves the right to make the final decision on how payment is made, and can withdraw the offer of scholarship if an amicable and realistic payment process cannot be reached.
Scholarships are awarded based on established eligibility criteria. You must meet the eligibility requirements to receive your scholarship payment. The eligibility requirements for each scholarship are outlined on the scholarship page of our website.
You may be permitted to hold more than one scholarship outside of the BLF Foundation.
The BLF Foundation is not responsible for and cannot provide you with financial advice on taxation matters. The information provided here is of a general nature only and does not take into account your individual financial circumstances. If you are enrolled full-time (at least 12 credit hours per semester), your scholarship payment may not be taxed, however the BLF Foundation is not responsible for checking and notifying you of, nor paying any taxation obligations.
Providing false or misleading information is an offense. If Resume Companion knows or has reason to believe that you have provided false or misleading information, Resume Companion may reassess your entitlement to the scholarship and you will be liable to re-pay to Resume Companion the total sum of any payments incorrectly made to you.
As an acceptance to the Mentorship Program, each applicant is required to attend 12 scheduled mentorship sessions. Each session will be scheduled on a weekend day once a month.
If the applicant cannot attend the session, email communication must be submitted at least three (3) calendar days before the scheduled session.
All Mentors agree to provide their mentorship services free of charge to the BLF Foundation. Mentor’s services are provided on a volunteer basis during the BLF Foundation Mentorship Program calendar year.
Healthy dialogue and exchange of information are encouraged for each applicant and Mentor pair. The applicant reserves the right to seek independent counsel before acting on any information, suggestion, advice or guidance given by the Mentor as part of the BLF Foundation Mentorship Program. All Mentor information, suggestion, advice or guidance will be acted independently by the applicant to the extent that if one does so, one shall do at the applicant’s own risk. The applicant hereby unconditionally and irrevocably waives any rights of action it may have against the Mentor in relation to any such information, suggestions, advice or guidance.
The applicant is advised to take independent financial, legal or other appropriate professional advice before acting on any information, suggestion, advice or guidance given by the Mentor.
The applicant acknowledges that any services provided by the BLF Foundation and the Mentor pursuant to this Agreement are provided free of charge and in good faith. Neither the BLF Foundation nor the Mentor will be liable to the applicant or to any third party for any loss, damage, costs or liabilities suffered as a result of this agreement, the existence of the relationships between the parties or the services provided. Nothing in this clause shall limit or exclude any liability for death or personal injury, or which results from fraud.
The BLF Foundation is not (and the applicant and the Mentor acknowledge that the BLF Foundation is not) liable for the services provided by the Mentor or for any acts or omissions of
This Agreement will continue in force for the duration of Mentorship class of the applicant or until terminated by any party by written notice to the others sent to the addresses given above.
While this Agreement is in force, the Mentor and the applicant will not either directly or through any intermediary enter into any other contract or arrangement as between them.
Each mentorship session will have required work that will be needed to complete. Each applicant will come to the mentorship sessions with a computer and/or tablet to complete assignments. There will be time during the session to work on assignments. It is required that these assignments are completed for mastery of the specific session content.
Each applicant will be paired with a mentor who is vetted by the BLF Foundation. This mentor will be available to provide guidance on the assignments and tasks but also will have ability to host various social activities with applicant.
The BLF Foundation will cover material costs for any items needed to complete the Mentorship Program. The BLF Foundation’s discretion will be utilized for any non-material costs for the Mentorship Program, including meals, travel, transportation, and other miscellaneous costs.
In consideration for being allowed to participate in any BLF Foundation mentorship activities that relate to travel, you hereby release, hold harmless and forever discharge the BLF Foundation and each and every officer, agent, and employee of each of the above referenced entities from all claims, causes of action, or demands of every kind which you may have in the future or that any person claiming through you may have in the future against any of them by reason of any injury to person or property, or death, in connection with your participation in the travel activity described below.
Further, you agree to indemnify each and every one of them for liability arising solely from your tortuous acts or omissions, and your assume the risk of traveling to and from the site of
You certify that you are in good physical health and physically able to participate in the described activity. You understand and acknowledge that serious accidents sometimes occur during activities such as this, and that some medical conditions may be exacerbated or aggravated, and that participants occasionally sustain mortal or serious personal injuries and/or property damage as a consequence thereof, and that your participation could result in loss of or damage to your property, serious injury to your body or to others and/or your death.
You are advised to obtain personal medical coverage. Furthermore, you agree to use your personal insurance to cover any medical expenses in the event of an accident or injury.
You will be informed of the risks and know the safety procedures to follow that are inherent in this travel activity.
In no way does this agreement constitute expectation for financial reimbursement to Mentors for participation in Mentorship activities or programs. Any financial reimbursement will be explicitly outlined in written communication to the Mentors prior to a scheduled activity. This is at the full discretion of the
At the end of the Mentorship Program, the BLF Foundation reserves the right to offer a financial incentive for full Mentorship Program completion. The evaluation criteria will be based upon attendance and activity during sessions. Successful completion could lead to an alumni volunteer position to help mentor a subsequent following class of applicants to mentor. The BLF Foundation reserves the right to provide a financial incentive for Mentorship Program alumni volunteers. After completion, alumni cannot receive financial incentives during the following years for Mentorship Program volunteering.
Applicants who received a scholarship from the BLF Foundation cannot reapply for any following years. Applicants who enter only the Mentorship Program will have the ability to apply for BLF Foundation scholarships for the year following the Mentorship Program.
The BLF Foundation also reserves the right to host a final year dinner to showcase the year of mentoring with financial sponsors. During the showcase, the BLF Foundation will require at least two volunteers to showcase knowledge learned via the Mentorship Program.
By signing this terms and conditions document, the BLF Foundation is granted through representatives and employees the right to take photographs and videos of you and my property in connection with the mentorship and scholarship program. You are authorizing the BLF Foundation, its assigns and transferees to copyright, use and publish the same in print and/or electronically.
You are in agreement that the BLF Foundation may use such photographs of you or videos of you with or without your name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
The BLF Foundation reserves the right to cancel the scholarship awards or Mentorship Program at any point. The BLF Foundation reserves the right to share your name, email, and resume with sponsors, BLF Foundation Mentorship Program speakers and affiliates.
The BLF Foundation reserves the right to make adjustments to this program and agreement. Any changes will be communicated in writing with a signed acknowledgment by the applicant.
